Mi prima se llama Carla. By saying "Tengo una prima" in the beginning, you are saying that YOU have a prima. So you stick with the you for this sentence.
Tengo una prima. MÍ prima se llama Carla.
Translation: I have a cousin. Mi cousin's name is Carla.
